New Patient Registrationregister_now

If you live within our practice area you are welcome to register.  Our reception staff will be happy to guide you through the procedure. You will need to provide proof of address i.e. 2 utility bills less than 3 months old. 

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a practice nurse which will include a blood pressure check and urine test. If you are on regular medication you will be required to make an appointment with a doctor.

Medical treatment is available from the date of registration. Please contact reception for further information.
