Online 'Pre-Registration' With The Practice
For all patients registering online please complete both the Pre-registration form and online medical questionnaire.
We do also require 2 proofs of address (less than 3 months old) i.e. utility bill, mobile phone bill, bank statement, tenancy agreement (for students - we will accept a letter from a landlord/college). You can either provide this information by emailing firstname.lastname@example.org or handing your paperwork to one of our receptionists who will take a copy. Please make sure you provide us with your proof of address within 7 days of completing your online registration.
All new patients will need to have a New Patient Check appointment with either our HCA or Practice Nurse please contact the surgery 48 hours after we have received all your relevant paperwork to make an appointment for this.
All paperwork should be completed in full to avoid any delay with your registration. If there is a problem we will contact you.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
