The Barnard Medical Group is moving to Clinical Triage
We are excited to share an important update that will improve how we deliver care to you and your family.
We have listened to your feedback, especially around long telephone queues and are making changes to address these concerns. As part of a national NHS initiative, GP practices across the country are moving towards a Modern General Practice model, designed to make accessing care easier, faster, and tailored to meet your needs.
The Barnard Medical Group will be adopting the above approach from 3rd November 2025, through the introduction of 'Clinical Triage'.
Clinical Triage is a system whereby every patient request is reviewed by a Doctor before an appointment is booked. This ensures that patients will receive the most appropriate care, based on their individual health needs.
Patients will be required to complete an online consultation form if they would like to book an appointment with a Doctor.
Once we have received your online consultation form, it will be reviewed by a Doctor, and a decision will be taken as to the most appropriate course of action.
When you submit an online consultation form you will be asked to provide a summary of your symptoms or reason for contacting the practice. One of our Doctors will then assess your request and decide the next step.
This might include;
A face to face or telephone appointment
Being signposted to another service (such as an Emergency Department, Urgent Care Centre, NHS 111 or a Pharmacist)
A message containing self-care advice
When completeing a online consultation form, please provide clear and detailed information as this will help the Doctor assess your needs accurately.
If more information is required, we may contact you with follow-up questions to help us to prioritise urgent cases.
Patients will no longer be able to book a Doctors appointment via the telephone or by attending one of our practice sites.
If patients are unable to complete an online consultation form or do not have access to a computer or smartphone, please contact the practice via telephone or attend one of our sites and a member of our team will complete it on your behalf.
Patients DO NOT need to complete an online consultation form if you would like to book an appointment with a Practice Nurse. Please continue to telephone the practice to book nurse appointments.
To complete an online consultation form - clink on the "Submit a new request" button below or visit the NHS App. Online consultation forms are available between 8am to 6.30pm Monday - Friday.



We provide GP and nurse appointments outside of our core opening hours on Saturday mornings and Thursday late evenings until 8pm (advance booking). We run a number of clinics for the management of diseases including asthma, COPD and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.
We are a Training Practice, training graduate Doctors for a career in General Practice.
We are a multi-disciplinary team in that we have Pharmacists, a Mental Health Practitioner, and a First Contact Physiotherapist who our patient population can see.

Both of our practice sites have reserved car parking spaces for the disabled positioned near our front entrances. Both our practice sites have automated electronic entrance doors and disabled toilets situated at ground floor level.
