Online 'Pre-Registration' With The Practice

We may also require two proofs of address dated within the last three months, such as a utility bill, mobile phone bill, bank statement, or tenancy agreement. For students, we will accept a letter from a landlord or college. You can provide this documentation either by emailing it to barnardmedicalgroup@nhs.net or submitting it in person to one of our receptionists at either site, who will make a copy. Please ensure this information is provided within 14 days of completing your online registration.

Please contact the surgery 48 hours after you have submitted your online form to schedule a new patient screening with our Healthcare Assistant. This brief health check includes a blood pressure assessment and a urine test and is available to all new patients. If you are currently on regular medication, you will also need to schedule an appointment with a doctor.
