Registering in Person

If you live within our practice area you are welcome to register at Barnard Medical Group. Our reception staff at either site will be happy to guide you through the process of registration.
To register with the practice you will need to complete a registration form.
Please see below, depending on the nature of the registration:
- Adults and children over the age of 16
- Registration Form (GMS1 Form)
- New Patient Questionnaire (Adults over 16 years)
- Children under 16 and previously registered at a GP surgery
- Registration Form (GMS1 Form)
- New Patient Questionnaire (Children under 16 years)
- 'New' baby registrations and not previously registered at any other practice
- 'New Baby' Registration Form
Filling out the questionnaire will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a new patient screening with our healthcare assistant (HCA) Maureen, this is a quick health check that includes a blood pressure check and urine test. If you are on regular medication you will be required to make an appointment with a doctor.
Medical treatment is available from the date of registration. Please contact reception for further information.
